Are you sure it's the right cable for the bike?

The Outside Sheath is getting pulled when you move the Handle Bars.

Set-up the Throttle Cable with the Forks pointing dead ahead.
Give the Cable a bit of Slackk at the Cable Slack Adjuster.

Seat the Cable by moving the Sheath firmly toward the Adjuster.
Secure the Sheath to the Frame with a Cable Tie or with a wrap of Duct Tape to anchor it from pulling on the return spring.

    That should do the trick.

With a hole in the sheath could the cable be sticking in sheath from rust?

I had a similar problem and my slack adjuster was all the way out.

it can be done, trim the plastic sheath back to the lengh you want with a sharp knife, very carefully snip the outer with side cutters, then with 2 pairs of long nose pliers unwind the coil.
